Requirements

 What are the requirements when processing documents?

If you are wondering what requirements you might be needing when processing your documents, or if in case you are out of the country and you need someone to process your documents instead of going back to do it personally which I am sure will cost you more or for instance, you do not have family, friends or relatives in Manila to assist you for the authentication of your documents, this post is for you;

If you need authentication of any of  the following documents, please take note of the following: 

1.  NSO Birth Certificate/CENOMAR/Marriage Certificate/Death Certificate

• If in case you received an NSO Copy with UNREADABLE, BLURRED, FADED entries, attach a photocopy of your Local Civil Registrar Copy of Birth Certificate
• VALIDITY:  CENOMAR will be valid only for 6 months from the date of issue.

2.  NBI Clearance 

• VALIDITY:  NBI Clearance will be valid only for 1 year from the date of issue.

3.  Police Clearance

• VALIDITY:  6 months from the date of issue

4.  LTO Certificate / Drivers License

• It's validity depends on the date of expiration of your Drivers License and not the date of authentication.
• You may secure an LTO Certificate and Drivers License Authentication, first at LTO Main Office in Quezon City.  This process will take 5 business days to get the certified LTO copy of Certificate and Drivers License.

5.  PRC Documents (Report of Rating/Board Certificate/License/Good Standing)

•  Bring all the original copy of this at any PRC Branch together with a CLEAR PHOTOCOPY of each and file for PRC AUTHENTICATION.  
• Why?  Because DFA DO NOT Authenticate ORIGINAL Certificates.  They authenticate Photocopies only of the original documents that are duly certified by the issuing government agency.  The original document will be returned to you.
• This process will take 3 to 5 business days.

6.  SCHOOL RECORDS     

Public/Government/Private Elementary  &  High School  

• Form 137/Certificate of Good Moral/Certificate of Graduation/Diploma 

          - Prepare a CLEAR PHOTOCOPY of each document from your School Registration Office and request to have it CERTIFIED (stamped/dry sealed).  Inform the school registration officer that you need a copy that is for DFA Authentication.

               - When presenting the photocopies, make sure you have the original copy with you.

               - Once certified by the school's registrar, bring the certified copies at DepEd covering your school location.  DepEd will attach endorsement forDFAAuthentication.

                -This procedure might take 3 to 4 weeks.  
            
Please take note also that NOT ALL SCHOOLS allows you to hand carry the documents personally at DepEd.  Hand carrying of documents normally allow only by PUBLIC/GOVERNMENT schools. Other schools will provide you only a CAV Claiming Stub which you can use after 4 to 5 weeks when claiming the document directly at DFA Aseana.

                
          - If you wish to make the processing in a shorter period of time, hand carrying of document is the best way.  You can ask the registrar if this would be possible.  Other schools, when you are asking for RUSH processing, they look for any proof of urgency.

College/University/State University

• Transcript of Records/Diploma/Certificate of Graduation

        -  Double check your Transcript of Records issuing purpose.  It must NOT be "For Board Examination Purpose Only,"  otherwise, you need to request a new original TOR from your school for employment or general purpose.

         -  Have all your original documents clearly photocopied and present it at the University Registrar
